The form includes:

· further information about your pet’s needs including exercise, food, dietary requirements, medication, your pet’s behaviour and interaction; and
· consent to publish photos on our Facebook page and Instagram (please note this is not a photo or update every day).
If you have a senior pet (aged 7 or over) we also require you to complete a "Senior Animal Form”, so we can ensure your older pet receives any additional care required.
